How does the registration in the ZSEER work?

Please note that a personal presentation at the ZSEER is not possible.
If you are not from the surrounding area, we recommend that you consult the SE Atlas to find a rare disease center close to your home.

Registration is done online at the BASE Network Portal of the Bavarian Centers for Rare Diseases at the following link:

There, click on the "Register" button of the Center for Rare Diseases Erlangen and follow the instructions.
Please be sure to enter your current e-mail address. After submitting the registration form, you will receive a confirmation e-mail.

After checking your registration (only during business hours ZSEER) you will receive three e-mails from us, each with an access code ("token"). Please check your spam folder if necessary. Please enter the code under the following link, which is also included in the e-mails:

The first access code leads to a detailed questionnaire about your complaints ("F2 admission form").

In addition, please send us the following documents by mail:

Please note that without these documents it is not possible to process your case for legal reasons.

The second access code leads to an internet page where you can upload your existing medical documents (doctor's letters, laboratory findings, imaging findings, imaging CDs, etc.) ("F3 upload form").

Please note that for optimal processing of your case, it is necessary to provide us with all medical records relevant to the case.

If you have already had contact with Erlangen University Hospital, another university hospital or another ZSE in Germany regarding your complaint, please wait for the full clarification and final response from the hospital or center and attach it.

The third access code leads to a questionnaire for your primary care physician ("F4-Epikrise HA").
You should either forward the email with this access code to your primary care physician as an email or print it out and hand it over personally with the request to complete the questionnaire.

By completing the questionnaires and sending us your documents, you agree that they may be viewed by our physicians.
